How It Works
Medicare Part B covers doctor visits, outpatient services, and medical equipment. Most people pay the standard premium, but higher-income beneficiaries pay an Income-Related Monthly Adjustment Amount (IRMAA) surcharge.

Monthly Premium = Base Premium ($185) + IRMAA Surcharge (if applicable)

Tips
- IRMAA is based on your tax return from 2 years ago (2025 premiums use 2023 income).
- If your income has dropped due to retirement or life event, file Form SSA-44 for reconsideration.
- Roth IRA conversions count toward MAGI — plan conversions to avoid IRMAA brackets.
- IRMAA is per person — both spouses pay it if income is above the threshold.
- The surcharge resets annually — a one-time income spike only affects one year of premiums.
